POSITION: Central Intake Worker
Centralized Intake Program
Full-time

POSITION SUMMARY: The successful candidate will work with individuals residing in Huron and Perth Counties. The role includes working with outpatients, completing comprehensive assessments to determine needs and facilitate referrals to appropriate mental health, addiction and/or community resources, and psychiatry. The Central Intake Worker will manage referrals, screen, assess, and refer in a timely manner to the appropriate outpatient programs as well as community organizations. They will ensure completed intakes are distributed to the appropriate program for further processing. The Intake Worker will make recommendations to the family and/or referring organization, regarding alternative services and assist them in linking with services. This role also includes identifying individuals requiring urgent psychiatric consultation and competency in expediting service delivery. This program will include seeing individuals at satellite locations as well as being able to respond to individuals requiring in home assessment. Staff will be expected to work in both Huron and Perth communities in this position as needed.

 

REPORTS TO: Manager, Outpatient Mental Health

DATE REQUIRED: As soon as possible

QUALIFICATIONS:
-Masters degree in Social Work, Occupational Therapy, or Registered Nurse is required
-Current registration with a regulated college in Ontario, such as OCSWSSW, OSPT, or CNO
-Current licensure with appropriate college or association
-Extensive knowledge of the Recovery Principles and Recovery Model
-Completion of Non Violent Crisis Intervention Training
- Completion of Non Violent Crisis Intervention Training
- Extensive knowledge of the Recovery Principles and Recovery Model
- Certification in Brief Solutions Focused Therapy preferred
- Completion of suicide training program preferred
- Completion of a crisis intervention certificate program preferred
- Current CPR certification

REQUIREMENTS:
- Minimum five years current progressive clinical experience working with individuals with a serious mental illness and/or addictions in a health care setting and/or community based setting
- Minimum three years experience completing comprehensive assessments and treatment plans with a variety of individuals in the community and/or hospital setting
-Extensive experience working with community partners and stakeholders in Huron and Perth Counties

SKILLS/ABILITIES:
- Demonstrated knowledge and competence in the application of treatment modalities to support individuals with serious mental illness and/or addiction
-Demonstrated ability to independently conduct formal mental health and addiction assessments, short term counseling, case management, and referral
- Knowledge of mental health reform and values, principles of recovery, Mental Health Act, PHIPPA, and Health Care Consent Act
- Organized and self-driven, highly skilled in collaborating with community agencies, have an interest in family intervention, a knowledge of psychotropic medications, and be competent in crisis intervention.
- Knowledge of treatment modalities and group facilitation is an asset
- Excellent assessment skills and development of treatment plans
- Demonstrated knowledge with psychopharmacology
- Demonstrated ability to work independently and within an interdisciplinary team
- Demonstrated strengths in interpersonal relations, problem-solving, written/verbal communication and conflict resolution
- Provide written and electronic reports in keeping with best practices and the standards of work
- Ability and willingness to attend satellite locations as well as being able to respond to individuals requiring in home assessment
- Self-directed, motivated team player with a commitment to ongoing professional development and client and family centred care
- Experience working with psychiatrists, family physicians and other interdisciplinary
- Willingness to travel throughout Huron and Perth Counties
- Willingness to work flexible hours to ensure that patient care and program needs are met
