BEHAVIOUR SUCCESS AGENT

TEMPORARY Part Time 0.5FTE (Approximately 8 months-ending on March 31, 2027)

 

THE OPPORTUNITY

GBGH is looking for a Temporary Part Time Accredited Health Care Professional for Behaviour Success with the Patient Flow team. This position will support the implementation of best practices for the assessment and management of older adults with cognitive impairment and responsive behaviours, with a particular focus on early intervention within the Emergency Department (ED). In addition to providing clinical leadership and mentorship to staff, the BSA Lead contributes to the improvement of care processes and the standardization of practices. Where capacity and time allow, the clinical scope of the role may expand to include broader geriatric assessment. The BSA Lead also identifies signs of caregiver burnout and ensures caregivers are connected to appropriate internal and external resources.

 

KEY RESPONSIBILITIES

  • Lead and support ED staff in the assessment and management of older adults with behaviors, focusing on early intervention strategies. by providing direct patient consultation and knowledge translation activities to hospital staff. Where capacity and time exists, the focus will expand to a broader geriatric assessment.
  • Collaborate with interdisciplinary teams to develop and implement evidence-based care pathways and standardized assessment tools for the ED.
  • Mentor and educate direct care providers through workshops, in-services, and case reviews.
  • Recognize signs of caregiver burnout during ED encounters; provide education, emotional support, and connect caregivers to internal and external resources (e.g., respite care, counseling, support groups).
  • Develop and distribute standardized information materials for caregivers, including burnout recognition and resource guides.
  • Participate in regional Communities of Practice and collaborate with Behaviour Support System partners to advance care in acute settings.
  • Contribute to the development and evaluation of site-specific sustainability plans for ongoing leadership and staff development.
  • Participate in the Behaviour Complex Case Resolution process as appropriate.
  • Update and implement the acute care NSM regional evidenced based best practice guideline for the care and management of older adults with cognitive impairment and responsive behaviour/personal expressions.

 

 KEY QUALIFICATIONS, SKILLS & EXPERIENCES

We are looking for a candidate who demonstrates the following attributes:

 

  • Accredited Health Care Professional in good standing with their College.
  • Bachelor’s degree preferred.
  • 2-3 years recent experience in direct care and/or clinical leadership.
  • GPA & PIECES training preferred.
  • Experience in developing care pathways, clinical tools, and education programs.
  • Proven knowledge in care of older adults with cognitive impairment and responsive behaviours.
  • Commitment to interdisciplinary care and evidence-informed practice.
  • Ability to collaborate, mobilize teams, and foster partnerships.
  • Strategic change management skills.
  • Decision-making, critical thinking, and conflict resolution skills.
  • Ability to work autonomously.
  • Proficient with computer programs like Microsoft Office Suite (i.e. Word, PowerPoint)

 

HOURS OF WORK

Temporary Part Time. Must be available to work shifts including days, evenings, weekends. Shifts and hours may change according to departmental requirements.

 

COMPENSATION

  • Hourly rate of pay $41.15 - $58.98
