Description

The role of the Discharge Transition Nurse (DTN) is to contact patients discharged from the hospital with a focus on discussion of the patients’ post-hospitalization plan of care. The calls do not provide medical advice. The calls may include suggestions and/or support to direct the patient to the appropriate resources or providers or to facilitate appointment scheduling. These suggestions or support are intended to assist the patient/caregiver to follow the plan of care identified in the patient’s discharge summary and Patient Oriented Discharge Summary (PODS).

During these calls the discharge transition nurse will use health literacy strategies including plain language and teach-back, the information contained in the patient’s PODS and a scripted dialogue to ensure that an appropriate discharge plan is in place and the patient/caregiver is able to follow this plan.

The phone calls also provide an opportunity to solicit information and collect ongoing data. Through ongoing experiential development this process may be modified over time to better meet the needs of the patient/caregiver. This position will work collaboratively with the Discharge Planner.

Qualifications

  • Current Registration with the College of Nurses of Ontario
  • Minimum 5 years varied/extensive clinical experience in medical/surgical, emergency and/or mental health nursing
  • Experience in case management is an asset
  • Demonstrated adaptability with the ability to perform effectively under stressful situations
  • Demonstrated analytical, problem solving, planning and organizational skills with attention to detail
  • Demonstrates initiative and strong judgment
  • Ability to work collaboratively with a team and independently with strong organizational and time management skills
  • Strong verbal and written communication skills with a focus on telephone skills
  • Strong computer skills
  • Knowledge of and ability to apply specific health literacy strategies (plain language and teach-back method) with patient and family-centered communication skills using motivational viewing techniques
  • Demonstrated interpersonal skills with the ability to work with an interprofessional team
  • Knowledge of the community services throughout Northwestern Ontario District
  • Ability to meet physical and mental requirements of the position
  • Reliable attendance record
